How Are You Feeling?

Your Symptoms Matter to your care team. Everytime you arrive for an appointment (except for daily radiation therapy, if required), you will answer online questions about how you are feeling using one of the kiosks in our waiting areas on each level. These online questions are called Your Symptoms Matter. Your answers tell us how you are feeling, so you and your care team can work together to monitor and manage your symptoms.

How to Complete Your Symptoms Matter

The short video below shows you how to use one of the kiosks to answer questions about how you are feeling. Please note, you will need your health card number to complete Your Symptoms Matters. If you need help with the kiosk, ask one of our volunteers in blue smocks.

Multi-language Support

If English is not your first language, check here for Your Symptoms Matter questions in 37 different languages to help you answer the questions at the kiosk. If you prefer a paper copy in your first language (if it is among the 37), ask a volunteer or other staff member for one.

Occasionally when you complete Your Symptoms Matter, you will be invited to complete an optional, five-minute, Your Voice Matters questionnaire. This questionnaire gives you the opportunity to share feedback on your most recent visit to the Stronach Regional Cancer Centre at Southlake or Southlake Regional Health Centre.
Your responses are kept confidential and help the Central Regional Cancer Program and Cancer Care Ontario make meaningful improvements to how they care for patients with cancer.
